Monday- Friday -12:00 PM-8:00 PM- holiday rotation- 50% in-office/50% in field

Home Healthcare experience preferred

- Ensures consistently high quality care to patients in accordance with the plan of care, policies, compliance requirements, procedures, and nursing best practices and professional standards. Supervises employees appropriately with timely and consistent coaching, feedback and reviews. Hires, trains, evaluates, and, when necessary, counsels or discharges evening and night nurses including evaluating performance through co-visits, chart audits and documentation review in the Agency's electronic medical record. Schedules and facilitates regular team meetings. Attends designated meetings for supervisory support. Ensures that staffing of nurses is appropriate to the caseload.
- Provides consistently high quality patient care in accordance with the plan of care and Agency policies. Provides initial and ongoing assessments to determine level of function, including OASIS assessments at appropriate time points. Prepares clinical and progress notes. Effectively manages a caseload equivalent to that agreed upon with the Director and regularly communicates with the scheduler regarding caseload status. Case manages patients seen on the evening shift, including doing regular visits to these patients and follows up to obtain plan of care orders for patients seen on evening/night shifts. Meets regulatory agency and pay source requirements to include goals, interventions, outcomes, frequency/duration of care, aide plans of care, and other required information. Handles medical emergencies calmly and quickly.
- Monitors documentation and reports of assigned field staff work and reviews for quality and compliance with regulatory and payer requirements. Facilitates communication between disciplines, verifying that paper or computer records are on file or submitted when needed, and coordinating proficiency of support team. Is proficient with the Home Care Home Base (HCHB) system and comfortable with related technology. Able to perform HCHB workflow as needed to support after hours operations.
4. Evaluates the effectiveness of existing services and identifies areas for updating expertise or developing programs. Ensures that staffing of nurses and support staff is appropriate to the workload.
