Job Description

Intake Nurse Care Coordinator is responsible for intake administrative support utilizing clinical judgement for patients referred to home health care. Provides patient care management services to ensure quality of care to those patients staffed by contract nurses.  (with or without reasonable accommodation) 



  • Evaluates referrals for completeness and eligibility criteria for home health care, consulting with physicians to obtain changes and/or clarifications to orders as needed. Acts as an on-going liaison between clinical managers, scheduling and admissions teams, field staff, hospital, Skilled Nursing facilities and physician office.

  • Supports Triage Nurses in assisting with triaging patient care needs, obtaining orders, responding to clinical concerns. Assists field staff with telephone calls and researching information as needed. Works closely with clinical managers when appropriate to resolve patient and staff problems.    

  • Provides case management to patients staffed by contract nurses, coordinating and revising care plan as appropriate.  Verified that visit documentation addresses patient needs according to plan of care and implementation of best practices.   Ensures regulatory compliance by contract nurses.   May complete the agency discharge visit to determine outcomes of plan of care as appropriate. 

  • Case conference with contract nurses and Agency nurse managers at minimum bi-weekly, documents progress toward plan of care goals and plan for discharge in EMR.  Serves as liaison to subcontracted agencies in the provision of nursing care.   Ensures through case conferencing that patients are receiving quality care appropirate to the plan of care.   May utilize customer service calls to determine patient satisfaction with care.

Education, formal training, certificates:


Graduation from an approved educational RN program. Must have an active Colorado RN license or a license from the state of primary residence, if participating in the NLC (Nurse Licensure Compact). Current BLS required.


Experience:


Minimum of 3 years recent Home Health experience requiring OASIS documentation and 1 year of hospital discharge planning experience required.


Knowledge, Skills and Abilities:



  • Demonstrates working knowledge of Medicare, Medicare Advantage and other non-medicare reimbursement requirements including eligibility for service and Veterans Administration pre-authorization process.

  • Ability to multi task, prioritize and manage time effectively.

  • Can read and interpret physicians' orders and notes as well as respond to them, verbally and in writing.

  • Can systematically obtain information and conduct clinical problem solving via telephone, e-mail or in person with other health care providers, patient, or family members.

  • Intermediate level proficiency with PCs in a windows environment. Efficient keyboard and accurate data entry skills for entering patient record information in an interactive healthcare database system.
