Admissions LPN ? with or without reasonable accommodation


Location: Colorado Visiting Nurse Association, 390 Grant Street, Denver, CO 80203.
Work Schedule:  Sunday- Thursday; (10AM- 7PM)40 hours.

As a Colorado VNA Admissions LPN, you will:



  • Answer calls during regular business hours, prioritize issues appropriately regarding clinical concerns and respond appropriately to emergency situations.

  • Process new or potential referrals, and accept faxed referrals from all referral sources, including taking additional orders from referral sources and accurately entering referral information on to appropriate forms.

  • Respond to persons who call during normal business hours and respond to questions.

  • Take appropriate action to meet client needs (e.g. processing faxes, emailing appropriate intra-agency departments).

  • Recognize and respond appropriately to emergencies, informing other of actions taken.

  • Interact closely with Supervisor to resolve situation.

  • Relay messages and reports off to any appropriate people per Agency procedure.

  • Act as a customer service representative to internal and external customers (patients, physicians, caregivers, co-workers and supervisors).

  • Communicate and respond to service requests by listening, clarifying, problem-solving and following up until resolution.

  • Applies knowledge of Agency resources to provide high quality customer service.

  • Suggests ways to improve triage process when warranted.


 


Minimum Qualifications:



  • Graduation from high school or equivalent.

  • Completion of an approved educational program for practical nursing.

  • Must have an active Colorado LPN license or a license from the state of primary residence, if participating in the NLC (Nurse Licensure Compact).

  • Twelve months nursing experience as an LPN and six months nursing experience in the last three years. Additional experience and/or other requirements may vary by case or department needs.

  • Anyone with less than 12 months hospital-based nursing may be subject to an extended probationary review period.
